Below is the registration form for our annual Walk and a donation form that can be used to donate to either the Walk or to the Swim-A-Thon, which was held in July at the Cheverly Swim and Racquet Club. (Donations to the Walk and Swim are tallied together and are gratefully accepted through Sept. 2009.)

Cost – Free registration, but tax-deductible donations are greatly needed and appreciated: Please be or sponsor a walker, using the form below to safely register and make a tax-deductible donation. To help us meet our goal it would be wonderful of each walker or family of walkers could donate at least $20-$25 or get pledges totaling at least that amount.

Please register by Sept. 12 if possible. You will also be welcome to register (and donate) at the event on Sept. 13, but we would prefer on-line registrations if possible.
